On-line Sample Submittal Form

One sample datasheet is required with each sample.

Be sure to include an MSDS for each product where available.

Remember that payment is due with the samples (either via credit card information or company check).

Currently we have two formats for submitting sample datasheets.

The on-line form allows you to enter redundant information only once (such as your contact information). When you hit the SUBMIT button, you will be given the option to return to the previous form already filled out, but omitting the previous sample designation number (i.e. your code number which will appear on the final report). Also, when you hit the SUBMIT button, the sample datasheet will be emailed to both of us. Submittal is via a secure internet connection to protect credit card data.

Alternatively you can print the form in a PDF format and submit one form along with each product.

Fill Out the Form Online. Secure access is provided so that you can include a credit card number and expiration date in the form.

PDF Format: print and submit with the sample(s). It is necessary to have Adobe Acrobat Reader installed.
